#cucukmyAZ hashtag had been appeared on social media for past one month and you know that is mainly related to opt in AStraZeneca Vaccination program which is going on now for people who opt for it at various location in Kuala Lumpur, Selangor, Penang, Johor and Sarawak. This is offering an opt-in choice for people who want to take the AstraZaneca vaccine.

Since the initial round of first-come-first-serve basis AstraZeneca Covid-19 vaccinations rolled out, thousands have taken to social media to flex their photo op booth pictures (hashtagged #CucukMYAZ for major bragging rights) and share their experiences from end-to-end. I done my part, receive the AstraVeneca Vaccine, and took a photo at the photobooth in purpose to encourage other to do their part, register for Covid-19 Vaccine.
Let me share about my experience of receiving the Covid-19 AstraZeneca Vaccine. My appointment is on 11:00am and is advice to be at the location/centre half hour before the appointment time (is reach at the centre, not reach at carpark, please be on time, do not make people wait for you!). If you’re worry about the infection risk, do wear double mask, or the golden standard N95 mask. However, If you’re super concern about the safety, please wear the face shield too.
You need to get ready your MySejahtera (Please do the daily self evaluation), and you need to show them that you’re Safe! (Please refer to the below for the details, either you’re low risk or casual contact). After the first check point, you need to proceed to another checkpoint, you need to show the appointment status to the officer, and in the status will mention which station to join. (eg. I’m station 2) Then all you need to do is follow the queue from Step one to Last step.
You will need to fill up and sign “Covid-19 Vaccination Consent Form” to affirm that you’re well understand about the vaccination and you understand that you might or might not have cause reaction & side effect. At the same time, by signing this consent letter, you agreed to complete the whole vaccination process and doses.
You will then giving Covid-19 vaccination by designated vaccinator. After received the vaccine, you need to proceed to another observation room, at that point you will be giving the vaccination card. You need to sit at this room for at least 15-30minutes, same time you’re listening to a briefing about do and don’t after vaccination,and also about the side effect you might be facing.
Now, everything is DONE, and you will see an update at your MySejahtera Vaccination Status. Please check your MySejahtera for the next dose date!

Classification of MySejahtera
|1||Low Risk||Individuals with low risk of COVID-19 infection.|
|2||Casual Contact||Individuals who were likely exposed to another person infected with COVID-19.|
|3||Close Contact||Individuals who have been exposed to other individuals infected with COVID-19. This includes: Health care associated exposure, including providing direct care for COVID19 patients without using appropriate PPE, working with health care workers infected with COVID-19, visiting patients or staying in the same close environment of a COVID-19 patient.
Working together in close proximity or sharing the same classroom environment with a with COVID-19 patient
Traveling together with COVID-19 patient in any kind of conveyance
Living in the same household as a COVID-19 patient
|4||Person Under Surveillance (PUS)||Individuals who are at risk of being infected with COVID-19. For example individuals with a history of traveling abroad within the last 14 days.|
|5||Suspected Case||Investigation (PUI) Individuals who are at risk of being infected with COVID-19, for example, individuals with a history of traveling abroad or have attended mass gatherings within 14 days before the onset of symptoms.|
|6||Confirmed Case||Individuals tested positive for COVID-19.|
